Yamhill County has one regular, full-time position for a therapist to work on the Assertive Community Treatment (ACT) team providing BH services for adults in a community setting. Duties include engagement, crisis support, diagnoses, direct treatment, performing integrated behavioral health assessments, service planning and implementation, individual/family therapy, case management, outreach/consultation for a caseload of adults with intensive behavioral health needs. Services are designed to assist individuals with a history of psychiatric hospitalization and/or at risk for hospitalization to live well in the community.
